Application to Work at Maple Bay Marina Non-Employee Tradesperson, Labourer or Workman

THIS FORM APPLIES TO ANY NON-EMPLOYEE WORKING ON MAPLE BAY MARINA PROPERTY. THIS APPLICATION MUST BE COMPLETED IN FULL BEFORE WORK COMMENCES. WORKING IN THE MARINA WITHOUT MARINA PERMISSION WILL BE REPORTED TO REGULATORY AUTHORITIES AND WILL BE CONSIDERED TRESPASSING.

The marina office must possess the following list of documents in order to process and/or approve an application. Work will not proceed without completion/submission of the following:

1. Application to Work at Maple Bay Marina (this document).
2. Maple Bay Marina must possess a current moorage agreement for the boat being worked on.
3. Proof of $2,000,000.00 liability insurance from the company, tradesman, labourer or workman
performing the work.
4. Copy of your Business License and proof of WorkSafe BC coverage (clearance letter).
